Blue Cross Minnesota offers many benefits to members that can help those who are caregivers as well as those who just need some extra help. Below is a guide to some plan benefits and information caregivers may find useful.

Just for Caregivers
Even family members need written permission from your loved one to get access or make changes to his or her health plan. Download and complete this Authorization for Disclosure of Health Information form to get started.

Services for Seniors
Choose a gym membership for access to fitness equipment and classes, or get home fitness kits designed for older adults. Learn more and register online.

Income Eligible Support Programs
If you have both Medicare Part A and B and receive Medical Assistance, you may qualify for SecureBlue, a Minnesota Senior Health Options (MSHO) plan. For eligible seniors, SecureBlue combines Medicare and Medical Assistance into one plan with no monthly premium.
