The single most important thing you can do to function efficiently and effectively as a caregiver is to create and maintain a comprehensive file on the person you care for. Whether it's electronic or on paper, in a box or a binder doesn't matter. What matters is that the system works for you.

Here's what you'll want to include (as relevant).

  • Medical history, including any surgeries or procedures
  • A current diagnosis
  • Physician contact information
  • Medication list (daily dosage, timing)
  • Pharmacy information
  • Allergies, especially to any medications
  • Calendar of doctors’ appointments
  • Insurance information (private, prescription plan, Medicare, Medicaid, long-term care, dental and vision)
  • Living will (may include advance directives)
  • Healthcare proxy (also known as durable power of attorney for health)
  • Financial power of attorney
